CO20: Daño cerebral perinatal

El grupo “Daño cerebral perinatal” es un grupo multidisciplinar en el que se integran neonatólogos, neurofisiólogos, radiólogos y psicólogos pertenecientes al Servicio Andaluz de Salud. Todos los componentes tienen actividad laboral clínico-asistencial, y 4 de sus miembros son doctores en Medicina.

El principal interés de nuestro grupo es el estudio de las bases neurobiológicas de las alteraciones del neurodesarrollo que se observan en recién nacidos de alto riesgo de padecerlas. Nos centramos en dos subpoblaciones de recién nacidos que presentan una mayor vulnerabilidad neurológica: aquellos recién nacidos a término expuestos a asfixia perinatal que desarrollan encefalopatía hipóxico-isquémica y los recién nacidos pretérmino. Empleamos distintas técnicas avanzadas de neuroimagen, fundamentalmente ecografía (ecografía cerebral 3D, power-doppler, etc) y de resonancia magnética (DWI, DTI), para el estudio del desarrollo de las distintas estructuras cerebrales. Estudiamos la asociación que distintos factores bioquímicos y clínicos y de nivel socioeconómico tienen en el neurodesarrollo.

Investigamos la eficacia de distintas medidas neuroprotectoras en estas dos poblaciones: en recién nacidos asfícticos diseñamos y participamos en ensayos clínicos sobre la eficacia que distintos fármacos (topiramato, alopurinol), asociados al empleo de la hipotermia terapéutica, tienen sobre el neurodesarrollo y en recién nacidos prematuros, tras desarrollar un modelo experimental de hemorragia cerebral en colaboración con un grupo de investigación básica de la Universidad de Cádiz, en la actualidad estamos estudiando el efecto que distintos fármacos (EPO, cafeína, etc.) tienen sobre la lesión cerebral y las secuelas neurológicas a largo plazo.

• Encefalopatía hipóxico-isquémica en el recién nacido a termino: Elaboración de un modelo predictivo de pronóstico en base a la neuroimagen, clínica y bioquímica.
• Neuroimagen en el recién nacido pretérmino: medidas lineales mediante ecografía 3D en el recién nacido pretérmino. Relación con el pronóstico. Comparación con medidas realizadas por RM. Comparación con medias fetales.
• Biomarcadores en la hemorragia intraventricular y en la hidrocefalia posthemorrágica del prematuro.
• Efecto sobre el sistema nervioso central de la infección perinatal por citomegalovirus y ureaplasma urealitycum.
• Modelo experimental murino de hemorragia intraventricular: fármacos neuroprotectores.
