IN59: Research Group on Chronicity, Continuity, and Quality of Care in the Province of Cádiz (CroniCA)
The IN59 Research Group on Chronicity, Continuity, and Quality of Care in the Province of Cádiz (CroniCA) aims to generate applied scientific evidence to improve care for people with chronic diseases, integrating quality of care, continuity of care, patient safety, perceived experience, and health outcomes. Its work addresses chronicity from a comprehensive perspective, incorporating aging, frailty, multimorbidity, institutionalization, health equity, health literacy, digital health, intersectoral collaboration, and the impact of climatic and environmental factors on disease progression and the use of health care resources.
Their Lines of research focuses on the development and evaluation of innovative models of integrated care, therapeutic optimization, health promotion, and the empowerment of patients and their caregivers, as well as on the organization of coordinated models that contribute to the sustainability of the healthcare system. Most of the group’s members work in primary care within the Campo de Gibraltar Oeste Health Management Area, which reinforces the group’s applied focus, its connection to clinical practice, and its ability to contribute to the planning and improvement of health and social-health services in the province of Cádiz.
